I am still investigating this.  The job actually has an 
"%%IncludeResource: procset CIDInit" line, doesn't it?  As a temporary 
fix for the CIDInit problem:

touch /usr/share/ppr/cache/procset/CIDInit-0-0

As for collated copies, what happens when you try to print collated 
copies?  How does the generated PostScript change if you try?
